Eligibility: All regular and long-term temporary Faculty and Staff with an FTE of 0.75 (30 hours per week) or greater are eligible the first day of the month following the effective date or date of hire.

- $670.00 per month - employee + spouse only
- $330.00 per month - employee + child(ren) only
- $1,275.00 per month - employee + family
Option 2: Kaiser - HMO
- $55.00 per month - employee only
- $550.00 per month - employee + spouse only
- $300.00 per month - employee + child(ren) only
- $1,060.00 per month - employee + family
Option 3: Anthem PPO-HSA
- $625.00 per month - employee only.
- $1,920.00 per month - employee + spouse only
- $1,350.00 per month - employee + child(ren) only
- $3,000.00 per month - employee + family
