Required Forms and Actions

IconThe following items must be reviewed, signed, and returned to Human Resources on or before your last day of employment.

  1. Final Paycheck Disposition Form

    This confirms how you will receive your final paycheck (direct deposit or pick-up).

    Required actions:

    • Sign by hand and return to Human Resources as soon as possible
  2. Check-Out Procedure

    These are the key steps for your transition.

    Required actions:

    • Return all university property (e.g., library materials, ITS equipment, keys, uniforms, ID card, student accounts, credit cards)
    • Collect required signatures from each department listed
    • Submit the completed form to Human Resources
  3. Notice to Employee as to a Change in Relationship

    This is the official notification of separation (i.e., resignation, retirement, layoff, discharge, etc.).

    Required actions:

    • Sign and date
    • Return the completed form to Human Resources
    • Keep a copy for your records
  4. Final Paycheck Acknowledgement

    This confirms you have received your final paycheck and that no further wages are owed. Requires your signature and mailing address for your W-2.

    Required actions:

    • Return the form to Human Resources
